The original company, Hospital Forms Services, was founded in 1978 by Wyman Dickerson. He created and printed forms, labels, envelopes, and several other printed materials for hospitals and other businesses throughout the valley. As time went on, his son Andrew Dickerson took over the company and kept it going through all the changes with the computer age coming in the 90s.

In 1997, Andrew created Forms Management Services, a DBA for Hospital Forms Services. He brought in Shelly to open up our market to apparel and promotional items as well as truck decals, banners, and more. In 2011, Penni came into the business with past printing experience, and we formed a great business relationship and friendship.

Andrew decided to retire in 2022 and Shelly and Penni took over where Andrew left off. Priding ourselves with exceptional client services, affordable and honest prices, and a personal, touch you can’t get very often in these times.

Shelly and Penni have over 40 combined years of experience in the printing, apparel, and promotion industries and can help you with any of your printing needs.
